Communication & Privacy Policy

I understand I am entitled to a copy of this notice upon request. By contacting BC Eyefitters PLLC by website contact form, email, and/or phone call, I give BC Eyefitters PLLC consent to communicate with me via phone, text and/or email regarding prescriptions, receipts, orders, insurance, and appointment information. I understand I can opt-out anytime by answering ‘STOP’ to the text message. 

BC Eyefitters respects your privacy and is committed to protecting your personal information. We do not share, sell, or disclose any information obtained through SMS text messages to any third party, except as required by law. All information you provide via SMS will be kept confidential and used solely for the purpose of communicating with you regarding our services, updates, or transactions. Your trust is important to us, and we take appropriate measures to safeguard your information.

Limited Warranty and Return Policy

All eyeglass frames are warranted for one year from original date of purchase with respect to manufacturing defects only.

In the event of a claim, BC Eyefitters PLLC reserves the right to repair or replace the frame and/or lenses at their discretion. Before a warranty claim can be processed, I understand that I will be notified by BC Eyefitters PLLC regarding any fees associated with such claim. Any fees associated with a warranty claim must be paid in full at the time of the claim.

I understand that the original frame and all of its parts must be returned in order to process a warranty claim. There are NO warranties on lost eyeglass frames, missing parts or lenses.

 Returns: There are no refunds or returns on custom-made eyeglass lenses, contact lenses, solutions or other merchandise. 

Patient Financial Responsibility (if using insurance for visits and hardware)

I hereby authorize BC Eyefitters PLLC to apply for benefits on my behalf for covered services rendered by them. I also assign my benefits and request that all payments from my medical and/or vision insurance provider be made directly to BC Eyefitters PLLC. I agree to assume responsibility for full payment pending any remaining balance that is not covered by my medical and/or vision insurance provider.

I understand that my insurance is not a guarantee of coverage and that my out-of-pocket expense may vary depending on factors including, but not limited to, benefit eligibility, deductibles, coinsurance and discounts.

Insurance benefits cannot be combined with any other discounts.

I certify that the information I have reported with regard to my coverage is correct. I authorize BC Eyefitters PLLC to release to my medical and/or vision insurance provider and its agents any information related to this or any related claim.
